Hospice CNA Job Description

Valeo Home Health & Hospice is a renowned healthcare provider in Utah, Salt Lake, Weber, and Davis counties. We are seeking a compassionate and skilled Hospice CNA to join our team.

Responsibilities:
  • Provide essential personal care duties for Hospice patients in their homes, including bathing, hair care, oral hygiene, nail care, foot care, skin care, toileting, and elimination.
  • Take accurate vital signs, including temperature, pulse, and respirations, and record them accurately.
  • Assist patients with transfers and ambulation as needed, and demonstrate knowledge of physical, emotional, and developmental needs of patients.
  • Support and maintain a culture of safety and quality, including basic infection control procedures, and recognize and implement emergency procedures as needed.
  • Identify and communicate findings to the supervising RN, and demonstrate the ability to chart according to professional standards.
  • Work as part of the interdisciplinary team, complete patient documentation on time, and attend in-services and staff meetings as needed.
Qualifications:
  • Graduate of an accredited High School or equivalent preferred.
  • Hospice Aide certificate obtained by Certified by State Department of Health Services.
  • Completion of a nurse aide training and competency evaluation program approved by the State as meeting the requirements of 483.151 through 483.154, and is currently listed in good standing on the State nurse aide registry.
  • Valid state drivers license and reliable automobile, current automobile insurance, and being willing to operate personal car(s) necessitated by nature of job.
  • Current TB testing and CPR card.
  • Listed on the nurse aide registry with no findings relating to client abuse, neglect, or misappropriation of client property.
