Job Summary

We are seeking a skilled Registered Nurse to join our team as a Case Manager. The successful candidate will be responsible for assessing, planning, implementing, and evaluating the care of patients across the continuum of care. This role requires strong clinical skills, excellent communication, and the ability to work collaboratively with the healthcare team.

Key Responsibilities

  • Assess patient needs and develop individualized care plans to ensure quality patient outcomes and appropriate utilization of healthcare services.
  • Coordinate care with the healthcare team to ensure a smooth transition from one level of care to another.
  • Provide ongoing support and expertise to patients and families to promote continuity of care and cost-effectiveness.
  • Collaborate with the healthcare team to integrate case management, utilization review, and discharge planning functions.
  • Ensure accountability for the care, coordination, and discharge planning of inpatient and observation services.

Requirements

  • Graduate of a National League for Nursing, Accreditation Commission for Education in Nursing, or Commission on Collegiate Nursing Education accredited school of nursing.
  • Licensed as a Registered Nurse by the Indiana Professional Licensing Agency.
  • Two years of recent clinical experience or three years of recent hospital case management experience.
  • Certification in Case Management (CCM or ACMA) is preferred.
  • Strong organizational and problem-solving skills, with the ability to function independently and interdependently as a member of a healthcare team.
  • Ability to establish rapport with a wide variety of people and demonstrate a willingness to try new approaches to problem-solving.
  • Strong computer skills.
