Job Title: Speech Language Pathologist - Skilled Nursing Facility

Job Responsibilities:

* Evaluate and assess speech, language, cognitive-communication, and swallowing disorders in residents of skilled nursing facilities (SNF).
* Develop and implement individualized treatment plans to address communication and swallowing deficits, enhancing residents' quality of life.
* Provide direct therapy services, including individual and group sessions, to improve speech, language, and swallowing functions.
* Educate residents, families, and caregivers on strategies and techniques to support communication and swallowing needs at home and in the facility.
* Collaborate with interdisciplinary teams, including physicians, nurses, occupational therapists, and physical therapists, to ensure comprehensive care for residents.
* Conduct regular progress assessments, adjusting treatment plans as necessary to meet residents' evolving needs.
* Document evaluations, treatment plans, progress notes, and discharge summaries in compliance with healthcare regulations and facility policies.
* Participate in care planning meetings, providing expert input on residents' communication and swallowing needs.
* Stay current with advancements in speech-language pathology, participating in continuing education and professional development opportunities.

Job Requirements:

* Master's degree in Speech-Language Pathology from an accredited program.
* Current state licensure or certification as a Speech-Language Pathologist (SLP).
* Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) is preferred.
* A minimum of 1-2 years of experience in a skilled nursing facility or geriatric setting.
* Strong knowledge of speech, language, and swallowing disorders, particularly in the elderly population.
* Excellent communication and interpersonal skills.
* Ability to work effectively as part of an interdisciplinary team.
* Strong organizational skills and attention to detail.
* Compassionate and patient-focused approach to care.
* Proficiency in electronic health records (EHR) systems.
