Job SummaryWe are looking for a skilled Hospice LPN to assist our RN Case Manager in carrying out patient care plans on an intermittent basis in patients' homes. The ideal candidate will have a strong commitment to quality care and a passion for making a difference in patients' lives.
Responsibilities- Assist the RN Case Manager in preparing, implementing, and evaluating patient care plans.
- Recognize and respond to the effects of social and economic problems on patients and their families.
- Prepare clinical and progress notes based on patient visits.
- Perform ongoing assessments during each visit and document data.
- Observe, record, and report patient symptoms, reactions, and changes to the appropriate person.
- Assist the RN in performing specialized procedures as directed by the physician.
- Participate in on-call duties as defined by our on-call policy.
- Current license to practice nursing in the Commonwealth of Virginia.
- At least one year of hospice experience required.
- Graduate of an accredited nursing school.
- Valid driver's license and reliable car available during work hours.
