Job Summary:

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at LifeStream, Inc. As an LPN, you will play a vital role in providing medical care and support to individuals with disabilities in their homes and communities.

Responsibilities:

  • Provide medical care and support to individuals with disabilities
  • Assist with daily living skills, including emotional and social support
  • Work collaboratively with the healthcare team to ensure high-quality patient care
  • Participate in seasonal farm share programs and company-sponsored activities

Requirements:

  • Current LPN license
  • Valid driver's license
  • Access to a vehicle
  • Ability to pass employment screening process
