Position Title: Physical Therapist for an In-house Skilled Nursing Facility

Employment Type: Part Time with a $3,000 Sign-on Bonus

Role Summary: The Physical Therapist will play a crucial role in the planning, organization, development, and direction of Physical Therapy services, ensuring compliance with applicable federal, state, and local standards, as well as guidelines and regulations. The goal is to maintain the highest quality of resident care.

Key Responsibilities:

  • Deliver physical therapy services to residents in alignment with established standards of practice and company protocols.
  • Follow company procedures for documentation and billing of therapy services.
  • Exhibit sound judgment in evaluating, planning, implementing, and following up on resident therapy programs.
  • Conduct regular screenings of residents to assess the need for intervention or treatment.
  • Evaluate residents to gather necessary data for effective treatment planning and execution.
  • Develop individualized treatment plans with clear short and long-term goals.
  • Communicate evaluation results, treatment plans, and recommendations to residents, families, and interdisciplinary team members.
  • Complete all assigned duties effectively and in the best interest of the organization.
  • Ensure compliant and medically necessary documentation in accordance with CMS expectations.

Qualifications:

  • Bachelor of Science Degree in Physical Therapy from an accredited institution.
  • Active Physical Therapy license in the state(s) of practice.
  • Commitment to ongoing education and professional development to maintain licensure and certification.
