Job Summary

We are seeking a highly skilled and dedicated Clinical Coordinator to join our team at Kaleidoscope Family Solutions, Inc. As a Clinical Coordinator, you will play a critical role in supporting individuals with disabilities by providing clinical management and oversight of professionals who work with them.

Key Responsibilities
  • Conduct comprehensive intakes with families and/or individuals to explain our services and ensure a smooth transition into our program.
  • Provide ongoing supervision and training to unlicensed direct care staff to ensure high-quality services are being provided.
  • Assign skilled Direct Support Professionals (DSPs) to ensure services are being delivered at the highest level.
  • Attend regular meetings with clients and their families to review progress and address any concerns.
  • Review staff notes and update as necessary to ensure accurate and up-to-date information.
  • Create and maintain clinically appropriate goals for individuals on your caseload.
  • Work collaboratively with families, internal teams, and colleagues to establish positive relationships and facilitate smooth transitions.
  • Utilize effective problem-solving skills and act as a model/mentor for others in demonstrating professional ethics and standards.
  • Supervise unlicensed staff to ensure they are meeting the needs of individuals on their caseload.
Requirements
  • Minimum of a Bachelor's Degree in Psychology, Social Work, or a related field.
  • Minimum of 1-3 years of experience working in human services conducting intakes, assessments, and in-home services.
  • Consistent access to a reliable vehicle.
  • Ability to travel or drive throughout the region as needed.
  • Self-motivated and independent worker with a flexible schedule.
