Job Title: Registered Nurse (RN) - Homecare Opportunities

Job Summary:

We are seeking a skilled and experienced RN to provide high-quality patient care in the comfort of their own homes. As a homecare RN, you will be responsible for assessing, planning, implementing, and evaluating patient care, as well as collaborating with our interdisciplinary team to ensure seamless care coordination.

Responsibilities:
  • Assess patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Provide skilled nursing care, including wound care, medication management, and vital sign monitoring.
  • Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care coordination.
  • Document patient progress and communicate effectively with patients, families, and healthcare team members.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current Registered Nurse (RN) license in the state of Kentucky.
  • Current Health Certificate (per facility RN contract or state regulation).
  • Current PPD or Chest X-Ray.
  • Current BLS card.
  • One year prior RN experience preferred.
