Key Responsibilities- Perform skilled nursing visits and coordinate client care to ensure optimal health outcomes.
- Act as the primary point of contact for all disciplines involved in patient care, ensuring seamless communication and coordination.
- Oversee the frequency of visits for each episode, ensuring timely and effective care.
- Consult with physicians and office staff as needed to provide detailed updates on patient care.
- Accurately collect and document OASIS data, ensuring medication profiles remain up-to-date.
- Ensure timely reporting of lab values to physicians.
- Attend weekly case conferences and monthly case manager meetings to stay informed and aligned with care plans.
- Current licensure as a Registered Nurse (RN) in the state of employment.
- A minimum of one year of clinical experience, preferably in home health or a related field.
- Demonstrated knowledge and skill in current nursing practice.
- A valid state driver's license and automobile liability insurance.
