Health and Social Services Coordinator Role

We are seeking a skilled Health and Social Services Coordinator to join our team at UnitedHealthcare. As a key member of our Integrated Care Model, you will play a vital role in ensuring our members receive the best possible care.

Key Responsibilities:

  • Assess and implement care strategies tailored to individual patient needs
  • Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
  • Manage care plans throughout the continuum of care as a single point of contact
  • Communicate with stakeholders to ensure quality coordinated care and services are provided
  • Advocate for patients and families to ensure their needs and choices are represented and supported

Requirements:

  • Current, unrestricted Social Worker license or LMHC in the state of Hawaii
  • 2+ years of social work or similar experience
  • Experience working directly or collaborating services for long-term care, home health, hospice, public health, or assisted living
  • Intermediate experience working with MS Word, Excel, and Outlook
  • Ability to travel in assigned region to visit Medicaid members in their homes and/or other settings

Preferred Qualifications:

  • CCM (Certified Case Manager)
  • Case Management experience
  • Experience with or exposure to discharge planning
  • Experience with utilization review, concurrent review, and/or risk management
  • Experience with electronic charting
  • Experience with arranging community resources
  • Field-based work experience
  • Background in managing populations with complex medical or behavioral needs
  • Background in mental health or experience working with serious mental illness
  • Bilingual in Cantonese or Mandarin
