The Registered Nurse (RN) in Home Health at Enhabit plays a pivotal role in delivering high-quality patient care. As a key member of our interdisciplinary team, you will be responsible for coordinating client care and performing visits to ensure effective treatment plans are implemented. With strong communication skills and a commitment to excellence, you will work closely with other healthcare professionals to provide comprehensive support to patients in their homes.
Key Responsibilities- Conduct patient visits and complete coordination of client care
- Oversee the frequency of visits for each episode
- Consult with physicians as needed to discuss patient care
- Perform accurate OASIS collection and ensure medication profiles remain current
To be successful in this role, you must be a registered nurse licensed in your state of employment. A minimum of one year of clinical experience is preferred, along with knowledge and skill in current nursing practice. Additionally, you will need a valid state driver's license and automobile liability insurance.
