Job Title: Community Health Worker

At Presbyterian Medical Services, we are seeking a highly motivated and detail-oriented Community Health Worker to join our team. As a Community Health Worker, you will play a vital role in assisting our Integrated Care Team in coordinating care for patients to achieve quality and cost-effective health outcomes.

Responsibilities:
  • Assist the Integrated Care Team in coordinating patient care to ensure seamless transitions and optimal health outcomes.
  • Proactively identify and address patient needs, providing education and support to promote healthy behaviors and self-management.
  • Collaborate with healthcare providers, community resources, and patients to develop and implement individualized care plans.
  • Provide emotional support and advocacy to patients and their families, promoting empowerment and self-advocacy.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
  • High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred.
  • Minimum one year experience in behavioral health, social services, healthcare, or education.
  • Strong verbal and written communication skills, with ability to work effectively with diverse populations.
  • Comfort with change and quality improvement processes, with ability to adapt to evolving healthcare landscape.
  • Experience with Electronic Health Records and Motivational Interviewing a plus.
