Key Responsibilities- Patient Care: Conduct initial and ongoing patient assessments, analyze assessment data, create plans of care, implement treatment, and evaluate treatment effectiveness.
- Medication Administration: Administer medications in a safe manner consistent with state and organizational policies and procedures.
- Care Coordination: Serve as a point of contact for patients' care coordination throughout hospital departments.
- Quality Improvement: Act as a patient safety advocate by participating in ongoing quality improvement initiatives in the department.
- Pharmaceutical Care: Provide comprehensive pharmaceutical care to a focused population and serve as a subject matter expert in the area of hematology/oncology.
- Education: Graduate of an ACPE-accredited School of Pharmacy with a BS or PharmD.
- Licensure: Maintains current licensure for pharmacist in the state(s) where practicing.
- Experience: 2 years of recent experience in an acute care setting.
- Advanced Degree: Advanced pharmacy degree, such as a Doctor of Pharmacy or Master's degree.
- Leadership Experience: 1 year of leadership experience.
