Role Overview
As we broaden our services in various markets, we are in search of a Patient Services Coordinator, Licensed Nurse who embodies our core values: Trustworthiness, Capability, Compassion, Proactivity, and a genuine calling to serve patients in their homes. This role acts as a vital link between patients and the necessary home health care services. You will be responsible for coordinating the intricate details among patients, providers, and our care teams to ensure timely and effective care transitions.
Key Responsibilities
- Schedule patient services and collaborate with patients, families, and care team members.
- Ensure proper scheduling of agency field personnel to maintain productivity and efficiency in care delivery.
- Provide essential information to field caregivers to facilitate appropriate patient care.
- Deliver care to patients in their homes as required.
- Ability to engage and collaborate with others to achieve team objectives.
- Capable of managing multiple challenges and competing priorities.
- Strong written and verbal communication skills.
- Accountable for delivering results within established timelines.
- Respond promptly to issues and challenges.
- Comfortable with technology; experience with HCHB is advantageous.
