Nexus Navigator

At Sierra Vista Child & Family Services, we are seeking a skilled Nexus Navigator to join our team. As a Nexus Navigator, you will provide critical support to individuals and families by connecting them with essential resources and services.

Key Responsibilities:

1. Provide access to advocacy, resources, and linkage to clients.
2. Offer outreach services to identify areas of need and set targeted goals.
3. Collaborate effectively with a multi-disciplinary team and community organizations.
4. Maintain accurate records and tracking systems.
5. Attend community events and promote a culture of quality and safety.

Requirements:

1. Bachelor's degree in Counseling, Social Work, or a related field.
2. Knowledge of community resources and bilingual skills preferred.
3. Valid California driver's license with less than two minor moving violations.

Essential Skills:

1. Effective communication and interpersonal skills.
2. Ability to work in diverse settings and maintain confidentiality.
3. Sound personal qualifications, including maturity and stability.
