Wound Care Specialist

At Hallmark Healthcare Center, we are seeking a dedicated and compassionate Wound Care Nurse to join our team. As a Wound Care Nurse, you will play a vital role in providing high-quality patient care and making a meaningful difference in the lives of our residents.
Key Responsibilities:- Assess, treat, and create care plans for patients with complex wounds, ostomies, and/or continence conditions.
- Collaborate with the nursing staff to provide comprehensive wound care services.
- Develop and implement in-service programs to educate nursing, rehabilitation, and medical staff on wound care best practices.
- Collect and analyze data related to wounds in the facility, and perform clinical evaluations for potential wound admissions.
- Must be a licensed RN or LPN in good standing with the state.
- 1-2 years of wound care experience required.
- Wound Care Certification (WOCN) preferred.
- 1+ years of long term care experience preferred.
