POSITION SUMMARY:
The Physical Therapist is responsible for evaluating and implementing treatment plans for patients requiring rehabilitation services. This role involves collaborating with a multidisciplinary team to ensure optimal patient outcomes.
DUTIES AND RESPONSIBILITIES:
** 1. Evaluates patient referrals for Physical Therapy services.
a. Conducts patient interviews and assesses physical conditions and rehabilitation needs.
** 2. Designs individualized treatment programs for patients.

Identifies appropriate therapeutic modalities and equipment usage, ensuring correct body positioning and technique during treatments such as therapeutic exercises, gait training, soft tissue mobilization, and electrical stimulation.
b.Educates patients on the proper use of assistive devices, including wheelchairs, crutches, and braces.
c. Adjusts treatment plans based on patient progress and feedback.d. Collaborates with physicians to discuss treatment alternatives when necessary.
** 3. Maintains comprehensive documentation in accordance with professional standards and departmental requirements, including evaluations, treatment plans, and progress notes.
** 4. Communicates effectively with nursing staff and interdisciplinary team members to coordinate patient care.
** 5. Provides mentorship and oversight to support staff involved in patient care.
** 6. Upholds departmental policies, procedures, and quality assurance initiatives, ensuring compliance with safety and infection control protocols.

EDUCATION:
Must possess or be eligible for a Physical Therapy license in New York State at the time of hire.
EXPERIENCE:
Previous experience in a healthcare setting is preferred.
CPR REQUIREMENTS:
Basic Life Support (BLS) certification is required within 90 days of hire.
PHYSICAL DEMANDS:
This position requires significant physical effort, including lifting and moving patients.
Must be capable of lifting a minimum of thirty (30) pounds independently and up to one-hundred and fifty (150) pounds with assistance.
EXPOSURE CATEGORY:
Category II. Tasks involve minimal exposure to blood or bodily fluids, though unplanned exposure may occur.
INFECTION CONTROL:
Adheres to universal precautions in all patient care activities.
