About the Role:

Kaiser Permanente is seeking a skilled Social Work professional to join our team as a Medical Caseworker. As a key member of our healthcare team, you will play a vital role in facilitating the coordination of appropriate, cost-effective services that are consistent with our members' plan of care.

Responsibilities:

  • Collaborate with the healthcare team to identify and address the psychosocial needs of our members.
  • Conduct thorough assessments to determine the most effective course of action for each member.
  • Develop and implement personalized plans of care that promote optimal independence and quality of life.
  • Provide financial counseling and referral services to help members navigate complex healthcare systems.
  • Access community resources and services to support members' needs.
  • Guide members through health-related legal processes and provide consultation and support to other healthcare professionals.

Requirements:

  • Masters degree in social work (MSW) required.
  • Experience with computer software programs in a Windows environment preferred.
  • Knowledge of community systems and resources in the defined service area preferred.
  • Knowledge of regulatory issues for the Mid-Atlantic area preferred.
