Registered Nurse Case Manager

Banner Health is seeking a skilled Registered Nurse Case Manager to join our team in Fallon, Nevada. As a key member of our healthcare team, you will play a vital role in providing comprehensive care coordination for patients across the healthcare continuum.
Job Summary:This position is responsible for assessing patients' plans of care, developing, implementing, and monitoring the utilization of resources and progress of the patient through their care. You will facilitate options and services to meet the patients' healthcare needs, ensuring optimal clinical, financial, operational, and satisfaction outcomes.
Key Responsibilities:- Manage individual patients across the healthcare continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes.
- Act in a leadership function with process improvement activities for populations of patients to achieve optimal clinical, financial, operational, and satisfaction outcomes.
- Collaboratively develop and manage the interdisciplinary patient discharge plan, effectively communicating the plan across the continuum of care.
- Evaluate the medical necessity and appropriateness of care, optimizing patient outcomes.
- Establish and promote a collaborative relationship with physicians, payers, and other members of the healthcare team.
- Collect and communicate pertinent, timely information to payers and others to fulfill utilization and regulatory requirements.
To be successful in this role, you must possess a current Registered Nurse (R.N.) license in the state of Nevada and a bachelor's degree in case management or healthcare. You should also have a working knowledge of care management, acute care, and/or home care environments, community resources, and resource/utilization management.
A minimum of 3-5 years of clinical experience is required, with a proven track record of critical thinking, problem-solving, effective communication, and time management skills. You must also be able to work effectively in an interdisciplinary team format and have a strong understanding of Banner Health's values and mission.
