Nurse Care Coordinator

We are seeking a highly skilled Registered Nurse to join our Senior Community Care team at Optum Home and Community Care. As a Care Team RN, you will play a vital role in providing high-quality care to our members in a facility setting.
Key Responsibilities- Ensure accurate and complete documentation of patient care
- Communicate and collaborate with the interdisciplinary care team to coordinate patient care
- Collaborate with the Patient Care Coordinator to support Advanced Practice Clinician care and scheduling of members
- Assist the provider/team with various care coordination activities, including benefit determination and therapy coordination
- Participate in facility-based meetings and schedule family conferences, team meetings, and team case presentations
- Coordinate training, tracking, and compliance with quality measures
- Assist in creating and conducting in-services for selected audiences
- Assist Providers in ensuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation
- Associate degree or higher in Nursing (RN)
- Active and unrestricted license in the state in which you reside, or ability to obtain by start date
- Certified in Basic Life Support
- Ability to complete physical requirements, including lifting a 30-pound bag and performing physical assessment
- Ability to travel between facilities
- Access to reliable transportation
- 1+ years of clinical experience in practice (acute care setting preferred)
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
- Saline Lock insertion experience
- Understanding of Geriatrics and Chronic Illness
