Nurse Care Specialist

We are seeking a skilled Nurse Care Specialist to join our team at JobRialto. The ideal candidate will have at least 24 months of experience as a Med Surg Tele RN in a hospital setting.
- As a Nurse Care Specialist, you will be responsible for performing comprehensive nursing assessments, diagnosis, planning, intervention, and evaluation, while coordinating care for patients and families across the healthcare continuum.
- You will ensure accountability for clinical, educational, quality, and fiscal patient care outcomes, following the care model established by the nursing department in accordance with agency policies, procedures, protocols, and standards of practice.
- Collaboration with the healthcare team is essential, reporting to the unit nurse manager.
- Perform comprehensive nursing assessments, diagnosis, planning, intervention, and evaluation.
- Coordinate care for patients and families across the healthcare continuum.
- Ensure accountability for clinical, educational, quality, and fiscal patient care outcomes.
- Follow the care model established by the nursing department in accordance with agency policies, procedures, protocols, and standards of practice.
- Collaborate with the healthcare team, reporting to the unit nurse manager.
- At least 24 months of experience as a Med Surg Tele RN in a hospital setting.
- Basic computer proficiency, including the ability to access, enter, and interpret computerized data.
- Strong interpersonal skills to facilitate communication with healthcare team members, patients, and families.
- Organizational skills to prioritize tasks and efficiently complete assigned work.
- Active license to practice professional nursing as a Registered Nurse.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required (AHA).
