As a Physical Therapist, you will play a vital role in helping individuals achieve their highest level of independence and bridging the transition from illness to wellness.

We are committed to providing a comprehensive array of community-based health and social service programs throughout western Pennsylvania. Our services include LIFE (Living Independence for the Elderly), skilled nursing, home health, personal care, adoption & foster care, assisted living, adult day services, memory care, hospice, palliative care, residential living, affordable housing, home safety monitoring, Meals on Wheels, rehabilitation services, and private duty.

Responsibilities
  • Provide quality Physical Therapy services, including resident/patient/client assessment, evaluation, care planning, treatment, and consultation in an interdisciplinary environment.
  • Support Home Health services in Slippery Rock, Grove City, Portersville, Mercer, Hermitage, and New Castle.
  • Collaborate with healthcare professionals to develop and implement individualized care plans.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
  • Provide education and training to patients, families, and caregivers on physical therapy techniques and strategies.
Qualifications
  • Bachelor's degree from an accredited four-year college or university with a degree in Physical Therapy; masters or doctoral preferred.
  • One (1) to two (2) years' related experience and/or training; or equivalent combination of education and experience.
  • Experience working with senior populations.
  • Working knowledge of computer software required for documentation and communication.
  • Maintains current PA license as Physical Therapist, including meeting continuing education requirements.
