Pinnacle Home Care Inc. is a leading provider of home care services, recognized for creating raving fans and delivering exceptional outcomes. We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Health LPN - Patient Care Coordinator.

Key Responsibilities:
  • Provide skilled nursing care under the direct supervision of the Registered Nurse (RN)
  • Perform selected skilled actions in the provision of curative, rehabilitative, palliative, or preventative nursing care
  • Responsible and accountable for making decisions based on individual's nursing experience and educational preparation
  • Timely completion of patient documentation
Requirements:
  • Current State LPN or LVN License
  • IV Certification
  • Graduate of an accredited practical nurse or vocational nursing program
  • One year of nursing experience, preferably in community/home health or medical/surgical settings
  • Possess and maintain valid CPR certification
  • Complies with accepted professional standards and practice
  • Demonstrates good verbal and written communication, and organization skills
  • Licensed driver with an automobile that is in good working order and insured
