About the Role

As a Home Health Nurse, you will be responsible for providing thoughtful care to our clients based on personalized Plan of Care. This includes providing in-home assessments, monitoring and tracking changes in the client's conditions and needs, and keeping the Director of Nursing and any other appropriate personnel informed.

You will also be responsible for proactively updating the client's family on plan of care, health changes, and concerns. We are looking for a nurse who is able to work independently and as part of a team to provide high-quality care to our clients.

Requirements
  • Have a current unrestricted RN license in the state of Utah
  • Certified CPR and negative TB skin test or chest x-ray
  • Have experience with skills including but not limited to vents & trachs, catheters, spinal cord injuries, hoyer lifts, and wound care
  • Have a minimum one (1) year of documented experience in a home health agency, primary care clinic, or health facility, preferred
  • Be an awesome driver with your own reliable vehicle and a valid driver's license
  • Have the ability to help our wonderful clients with mobility by providing varied assistance
