Rainbow Community Care is seeking a dedicated Registered Nurse to join our team at our Inpatient Center in Johnson Creek, WI. As a key member of our care team, you will provide high-quality care to terminally ill patients and their families.
Key Responsibilities- Assess patients' physical, psychosocial, and environmental needs, and develop individualized care plans to meet those needs.
- Provide education and support to patients and their families, and coordinate care with other healthcare professionals as needed.
- Manage pain and symptom management, and provide end-of-life care in accordance with county and state regulations.
- Collaborate with the care team to ensure seamless delivery of care, and demonstrate flexibility in work schedules as needed.
- Wisconsin Nursing license required.
- Hospice experience preferred.
- Valid driver's license and current auto insurance required.
