CenterWell Home Health is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Nurse Specialist. As a key member of our care team, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.
Responsibilities- Provide admission, case management, and follow-up skilled nursing visits for home health patients.
- Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
- Coordinate care with various healthcare personnel, including Physical Therapists, Occupational Therapists, and Home Health Aides.
- Report patient care/condition/progress to patient's physician and Clinical Manager on a continuous basis.
- Implement patient care plans in conjunction with patients and families to assist them in achieving optimal resolution of needs/problems.
- Prepare medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes.
- Diploma, Associate, or Bachelor Degree in Nursing.
- Minimum of one year nursing experience preferred.
- Strong med surg, ICU, ER, acute experience.
- Home Health experience a plus.
