Job SummaryThe Ambulatory Care Nurse (ACN) is a Registered Nurse responsible for providing nursing care management services to assigned populations. This role involves a collaborative approach to anticipate, integrate, and coordinate patient care plans, maximizing the ability to remain in a community setting.
Key Responsibilities- Identify at-risk patient populations requiring care management services in collaboration with physicians.
- Serve as a patient/family advocate and act as a communication link between the physician, patient, family, and interdisciplinary healthcare team, as well as community resources.
- Support the mission, values, and management of patient care services at WakeMed.
- Actively support and incorporate ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.
- 2 Years Nursing - Related Area Or No Experience Required With Preferred Education.
- Diploma Nursing Or Associate's Degree Nursing Required - And Bachelor's Degree Nursing Preferred.
- Registered Nurse Required.
Hours vary, with the office open Monday through Friday from 7:00am to 5:00pm and Friday from 7:00am to 3:00pm. No weekends or call requirements.
