Discharge Planner

BayCare Health Systems is seeking a skilled Discharge Planner I to join our team at Northbay Recovery Center. As a key member of our care team, you will play a vital role in ensuring seamless transitions for patients with acute mental health and substance abuse issues.

Key Responsibilities:

• Assess and develop individualized discharge plans to address patient needs and facilitate successful transitions.
• Collaborate with multidisciplinary teams to ensure comprehensive care and effective linkage with community resources.
• Monitor and advocate for patients' rights and interests, ensuring compliance with regulatory requirements and best practices.
• Provide education and support to patients and families, promoting self-advocacy and empowerment.

Requirements:

• 1+ year of experience in behavioral health or acute care settings.
• Bachelor's degree in a related field (e.g., counseling, health education, social work, psychology, nursing).
• Master's degree in human services preferred.
• BLS (Basic Life Support) certification.
