Responsibilities
- Evaluate physician medication orders for appropriateness, potential interactions, and route of administration, making recommendations for therapy changes as needed.
- Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medications.
- Collaborate with the healthcare team to provide clinical and technical support, ensuring that laboratory tests for monitoring high-risk conditions are ordered, evaluated, and discussed with the team.
- Document all communication with physicians, nurses, and other healthcare team members in the medical record, ensuring accurate and timely documentation.
- Implement policies and procedures for the safe procurement, storage, distribution, and use of drugs and biologicals, as well as disposal of drugs.
- Inform the Pharmacy Manager of medication-related errors and assist facility/agency staff with procedures relating to safe and effective methods of procuring, maintaining, administering, and disposing of drugs.
Qualifications
- Bachelor's Degree in Pharmacy; Pharm D preferred.
- One year experience in long-term care.
- Licenses/Certifications: As required by government regulations and business needs.
- Detail-oriented, goal-oriented, team player, and excellent oral communication skills.

Salary Range
USD $124,800.00 - $128,960.00 per year.
