We are seeking a skilled and compassionate Home Health Skilled Care Provider to join our team at LHC Group. As a Licensed Practical Nurse (LPN), you will be responsible for providing quality patient care and performing technical skilled care in the patient's home.
About the Role:
As a Home Health Skilled Care Provider, you will work in accordance with the plan of care, in coordination with other members of the healthcare team, and in accordance with the state laws governing the practice of nursing. Your responsibilities will include instructing the patient's family in the care of the patient and maintenance of a healthy environment for the patient, observing, recording, and reporting to the supervising nurse and/or physician the patient's response to treatment and changes in the patient's condition daily.
Requirements:
To be successful in this role, you must have current LPN licensure in the state of practice, a current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation, as well as CPR Certification.
