POSITION SUMMARY:
This role is dedicated to providing nutritional services and education to patients undergoing dialysis and their authorized caregivers within a clinical environment. The primary goal is to enhance the nutritional health of patients suffering from end-stage renal disease, adhering to the established policies and regulations of LifeBridge Health.

KEY RESPONSIBILITIES:
1. Conduct comprehensive nutritional assessments to identify appropriate dietary interventions and recommendations.
2. Perform initial assessments within the designated timeframe following patient admission or transfer.
3. Reassess patients periodically, with specific timelines based on their stability.
4. Collaborate in interdisciplinary care planning, ensuring timely updates based on assessments.
5. Review and document patient medical and dietary histories, maintaining accurate records of nutritional progress.
6. Develop tailored Medical Nutrition Therapy plans, including meal planning that meets individual patient needs.
7. Provide nutrition education and counseling, taking into account the patient's medical, economic, and social circumstances.
8. Monitor and evaluate patients' nutritional status, laboratory results, and adherence to dietary recommendations.
9. Update nutritional care plans as necessary and communicate findings with the healthcare team.
10. Engage in quality improvement initiatives and participate in team meetings to enhance patient care.

QUALIFICATIONS:
- Bachelor's degree in Nutrition, Dietetics, or a related field.
- Registered Dietitian credentialing and state licensure required.
- A minimum of two years of clinical nutrition experience as a Registered Dietitian is essential.
- Preferred experience includes five years in a dialysis setting.
- Strong communication, critical thinking, and teamwork skills are necessary for success in this role.
