Job Summary

We are seeking a highly skilled Behavioral Health Therapist to join our team at Mid Michigan Health. As a key member of our primary care treatment team, you will play a vital role in identifying, assessing, and managing patients with medical and behavioral health problems.

Responsibilities
  • Assess the clinical status of patients referred by primary care providers and intervene to attenuate further psychological or physical deterioration.
  • Assist primary care providers in recognizing and treating mental disorders and psychological problems, and consult with physicians as necessary.
  • Facilitate patient engagement and follow-up care, and work with the primary care team to treat and manage patients with chronic emotional and/or physical health problems.
  • Provide brief behavioral interventions using evidence-based techniques, such as behavioral activation, problem-solving treatment, and motivational interviewing.
  • Document treatment response and monitor patients for changes in clinical symptoms and treatment side effects or complications.
  • Improve clinical outcomes with high prevalence medical and mental health conditions, and connect with new and established patients regarding integrated care model and care team information.
  • Evaluate patient care plans with the primary care team and develop appropriate goals for behavioral health conditions.
  • Perform coding and billing following appropriate guidelines, and track patient follow-up and clinical outcomes using a registry.
Requirements
  • Masters Degree in Social Work or related field.
  • Licensure as a Licensed Psychologist or Master's Social Worker in the State of Michigan.
  • Membership in at least one professional organization, such as the American Psychological Association or National Association of Social Workers.
  • Training and experience in casework principles, including diagnosis, assessment, crisis intervention, treatment, and transition planning.
  • Knowledge of Integrated Behavioral Health practices, processes, and procedures, as well as interpersonal skills needed to communicate successfully with individuals and groups.
Working Conditions

This position requires the ability to work in a fast-paced environment, handle multiple tasks, and maintain regular and predictable attendance. The successful candidate will also be able to stay awake and alert during work hours.
