Job Summary

The LPN will be responsible for providing direct patient care, administering medications, and performing various medical procedures. You will work closely with our interdisciplinary team to assess resident needs, develop care plans, and implement interventions to promote optimal health outcomes.

Key Responsibilities
  • Provide direct patient care, including vital sign monitoring, wound care, and medication administration.
  • Assess resident needs and develop individualized care plans to meet their unique requirements.
  • Collaborate with the healthcare team to implement interventions and evaluate their effectiveness.
  • Administer medications and start intravenous fluids as prescribed by physicians.
  • Document resident information, including medical history, treatment plans, and progress notes.
  • Participate in quality improvement initiatives to enhance patient care and safety.
Requirements
  • Current LPN licensure in the applicable state.
  • CPR certification upon hire or obtained during orientation.
  • High school graduate or equivalent, with completion of an accredited LPN program.
