Job Title: Psychiatric Rehabilitation Worker

We are seeking a dedicated and compassionate Psychiatric Rehabilitation Worker to join our team at RHA Health Services. As a key member of our team, you will play a vital role in providing support to individuals with mental health or co-occurring issues in a site-based setting.

Job Summary:

The Psychiatric Rehabilitation Worker will provide rehabilitative services, including recovery, stigma, health, and wellness, and personal development. This role will involve developing, implementing, monitoring, and revising the Functional Assessment and Person-Centered Plan. The successful candidate will also support individuals in developing various skill-building activities, monitor service utilization, and provide counseling and guidance to individuals to effectively deal with problems and develop positive relationships.

Key Responsibilities:
  • Provide rehabilitative services, including recovery, stigma, health, and wellness, and personal development.
  • Develop, implement, monitor, and revise the Functional Assessment and Person-Centered Plan.
  • Support individuals in developing various skill-building activities.
  • Monitor service utilization and ensure it is effective, appropriate, and aligned with the individual's identified Person-Centered Plan.
  • Provide counseling and guidance to individuals to effectively deal with problems and develop positive relationships.
  • Assist with access to community resources in accordance with the individual's personal goals for community involvement.
  • Prepare and complete all necessary documentation, including 90-day reviews, activity notes, procedure documentation for admission, incident reports, continued stay information, and miscellaneous paperwork/reports within required timeframes as requested.
  • Utilize safe crisis management techniques to manage situations involving agitated and/or hostile behaviors, without the use of physical restraints.
  • Develop and maintain positive relationships with key stakeholders, ensuring stakeholder satisfaction with the company by maintaining proactive, responsive communication.
  • Direct communication with management and executives, establishing and maintaining frequent and regular communication with corporate executives.
  • Identify and execute on growth opportunities, including the development of supports and services that are in alignment with the company's service array and mission.
Requirements:
  • Complete criminal background check.
  • Name checked in the registries (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities).
  • Drug testing.
  • Education verification and other credentialing based on position requirements.
  • Proof of employment history or references (if required).
  • Positions that require driving: Proof of driver's license, driver's insurance, and vehicle (if required for providing transportation for individuals).
