Job Summary

We are seeking a highly skilled RN Patient Care Navigator to join our Neurosciences team at Northwest Community Hospital. As a key member of our care team, you will provide exceptional patient care and navigation services to patients and their families throughout their healthcare journey.

Key Responsibilities
  • Act as a primary advocate for patients and families, ensuring they understand their diagnoses, treatment strategies, and recommended plans of care.
  • Empower patients and families to communicate concerns, preferences, and priorities to their healthcare team.
  • Facilitate patient entry into our program and coordinate their care, following them throughout the continuum of care services.
  • Establish a coordinated plan of care with goals, interventions, and treatment plans based on patient needs, preferences, and values.
  • Assess educational needs of patients and caregivers, providing clinical education and information on available options.
  • Coordinate patient care and serve as a liaison between the care team, communicating with multidisciplinary teams as needed.
Requirements
  • Registered Nurse with a Bachelor of Science Degree in nursing or completion of an accredited Physician Assistant program.
  • Current Illinois licensure to practice nursing or current Illinois Physician Assistant license.
  • Three (3) years nursing or Physician Assistant experience in assigned specialty field.
