This role involves providing comprehensive care in skilled nursing facilities (SNFs), focusing on patient consultations and ongoing rehabilitation support.
Key Responsibilities:
1. Conduct initial consultations and follow-up assessments for patients in rehabilitation settings.
2. Regularly round on patients to monitor progress, address barriers to recovery, and enhance overall quality of life.
3. Maintain an active caseload, ensuring all referrals are captured and managed effectively.
4. Collaborate with clinical staff and facility leadership to foster a professional and collegial environment.
5. Engage with patients and their families, clearly communicating treatment options and expectations.
6. Participate in facility rehabilitation team meetings to discuss patient care strategies.
Documentation and Collaboration:
Adhere to established protocols for documentation, billing, and coding. Maintain open lines of communication with facility leadership to understand their needs and provide updates on patient care.
Qualifications:
- Unrestricted NJ Medical License and CDS, DEA
- Board Certified or Board Eligible in Physical Medicine and Rehabilitation
- Commitment to ongoing education and compliance with licensing requirements
Schedule and Compensation:
This position offers flexibility with options for full-time or part-time work. Compensation ranges from $200k to $300k, based on the desired caseload, with additional performance-based incentives. Benefits include health, dental, and retirement plans, along with ongoing support from a dedicated administrative team.
