Job Overview

We are seeking a skilled Registered Nurse to join our team as a PC Nurse. This role will involve providing skilled nursing care to patients in their homes, under the guidance of a plan of care/treatment.

Key Responsibilities
  • Confer with physicians to develop initial plans of treatment based on patient assessments and physician orders.
  • Provide hands-on care, management, and evaluation of care plans, as well as teaching patients in accordance with physician orders.
  • Revised plans in consultation with physicians based on ongoing assessments and policy/regulation requirements.
  • Admit patients to services, completing clinical documentation under the Clinical Manager's direction.
  • Coordinate and supervise the work of Home Health Aides, Certified Home Health Aides, and Personal Care Workers, providing written personal care instructions/care plans that reflect current plans of care.
Requirements
  • Bachelor's or Associate's degree in Nursing or equivalent.
  • RN licensure.
  • Proof of current CPR certification.
  • At least six months of professional nursing experience as a Registered Nurse.
