Job Summary
We are seeking a skilled Admission RN to join our team. As an Admission RN, you will be responsible for assessing patients in their place of residency or in hospitals and initiating care into the services of Chapters Health System.
Key Responsibilities
- Conduct thorough assessments of patients to identify their needs and develop personalized care plans.
- Collaborate with healthcare professionals to ensure seamless transitions of care and effective communication.
- Provide education and support to patients and their families to promote understanding and empowerment.
- Ensure compliance with regulatory requirements and Company policies.
- Develop and maintain strong relationships with healthcare providers and facilities to facilitate referrals and care coordination.
Requirements
- Bachelor's degree in nursing or related field.
- Current Florida license as RN, current active single-state out-of-state nurse license, or current active multistate/compact nurse license.
- Minimum of two (2) years of RN experience in acute care, hospice, home health care or other post-acute care.
- Strong clinical assessment skills with preferred background in emergency department or ICU.
- Ability to effectively communicate in English (verbal and written); bi-lingual in Spanish preferred.
