About the Role

We are seeking a highly skilled and detail-oriented Care Coordinator to join our team at Nascentia Health. As a Care Coordinator, you will play a critical role in providing administrative and technical support to our Care Coordinator Supervisor and team members.

Key Responsibilities
  • Provide accurate review and entry of authorization data into a computerized database.
  • Manage expiring authorization reports and assist case management with scheduling and coordination of patient services.
  • Communicate with Members on a regular basis and collaborate with team members to support Members' needs.
  • Act as a resource to Care Managers, assist with scheduling and tracking of Member contacts, visits & UAS, and answer phone calls for the team.
  • Perform clerical duties, such as filing, typing, copying, faxing, and taking messages, as directed by supervisor.
Requirements
  • High school diploma; Associate's degree preferred in a related field.
  • 2 or more years' experience in the healthcare industry or a degree as a Medical Assistant.
  • Strong computer literacy and comfort with industry standards.
  • Proven analytical and problem-solving skills to identify potential issues and participate in their resolution.
  • Proficiency with computer systems, working knowledge of Microsoft Office, Internet, and various software applications.
  • Ability to work independently and collaboratively, with the ability to adapt to shifting priorities, demands, and timelines.
  • Demonstrate discretion in handling confidential and sensitive information.
  • Excellent communication skills, verbal and written.
Physical Requirements
  • Ability to sit or stand at a desk 90% of the day.
  • Frequent sitting, standing, walking.
  • Visual/hearing ability sufficient to comprehend written or verbal communication.
  • Ability to express oneself verbally and in writing.
  • Ability to use the telephone and communicate over the phone.
  • Extended periods of computer use.
  • Ability to lift and move objects up to 40 pounds.
