We are seeking a skilled Physical Therapist to join our team in a Home Health setting. The ideal candidate will have a strong passion for delivering high-quality care to patients in their homes.

Key Responsibilities:

  • Evaluate patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions.
  • Develop and implement individualized treatment plans to improve patient well-being.
  • Work collaboratively with the healthcare team to ensure comprehensive care.
  • Communicate effectively with patients, families, and healthcare professionals.

Requirements:

  • Current Physical Therapy licensure in state of MA.
  • Current CPR certification.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
