As an Independent Optometrist leasing space inside Walmart stores, you will play a key role in patient care, focusing on eye and vision health, as well as general well-being.

Key responsibilities include:

  • Establishing your own practice schedule and hours of operation
  • Controlling patient schedules and setting fee structures
  • Opting into equipment, instrumentation, and administrative support services (if permitted by state regulations)

To be eligible for a lease agreement, you must meet the following minimum qualifications:

  • Hold a Doctor of Optometry (OD) degree from an accredited optometry school
  • Be licensed to practice optometry in your state
  • Have a General Liability Certificate and Professional Liability Insurance Certificate
  • Provide a W-9 form
