Job Summary

We are seeking a highly motivated and compassionate Family Support Specialist to join our team at Scott Carver Dakota CAP. The successful candidate will be responsible for providing case management services to individuals and families experiencing homelessness, with a focus on supportive housing and stability.

Key Responsibilities:

  • Assess client needs and develop individualized plans to achieve housing stability and self-sufficiency.
  • Provide housing search and placement assistance, as well as ongoing case management and support.
  • Collaborate with community partners and internal teams to ensure comprehensive services and support.
  • Monitor client progress and adjust plans as needed to ensure successful outcomes.
  • Develop and maintain relationships with local landlords and service providers to access housing resources.

Requirements:

  • Four-year degree in social work or related field, plus one year of experience working with homeless populations.
  • Knowledge of applicable housing policies, procedures, and regulations.
  • Experience working with mental health and chemical dependency populations.
  • Specialized software applications and procedures, including HMIS.
