As a Case Manager at Family Service League Inc., you will play a vital role in supporting our Home Base program's mission to empower youth and families to overcome challenges and achieve their goals.

Job Summary

This full-time position requires a skilled and compassionate individual who can provide direct service, counseling, and guidance to our clients. The successful candidate will have excellent communication and interpersonal skills, knowledge of community resources, and the ability to navigate complex systems.

Duties and Responsibilities
  • Conduct home visits to assess client needs and develop personalized service plans.
  • Provide linkages, coordination, advocacy, and mentoring services to youth and their families.
  • Collaborate with other agencies and organizations to provide comprehensive services to our clients.
  • Document all client interactions and maintain accurate records.
  • Participate in agency-wide initiatives and collaborative projects.
Requirements and Qualifications

To be considered for this role, you must have:

  • A bachelor's degree in social work or a related field.
  • At least 2 years of experience working in human services.
  • Excellent communication and interpersonal skills.
  • Knowledge of community resources and ability to navigate complex systems.
  • Flexibility and adaptability in a fast-paced environment.
  • Proficient computer skills, including Microsoft Office.
  • A valid and clean NYS Drivers License.
