We are seeking a highly skilled Registered Dietitian to join our Home Care team. As a key member of our team, you will be responsible for providing clinical nutrition assessment and documentation for patients with chronic diseases in their homes.
Key Responsibilities- Conduct comprehensive nutrition assessments and develop personalized care plans for patients with chronic diseases.
- Maintain quality assurance and evaluate compliance with corporate policies and procedures, as well as State and Federal guidelines.
- Collaborate with healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Bachelor's degree in Nutrition and Dietetics, Home Science, or a related field.
- Licensed with the State of New Jersey as a Registered Dietetic Technician or Registered Dietician or Dietary Manager Association or as applicable.
