Job Title: Licensed Practical Nurse, Night Shift

Job Summary:

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at the Indiana Veterans' Home. As an LPN, you will provide high-quality nursing care to our residents, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities:

  • Administer medications and treatments in accordance with established standards and protocols.
  • Provide safe and efficient personal care to residents, including bathing, dressing, and grooming.
  • Use recommended safety devices and follow infection control standards to prevent the spread of illness.
  • Answer call lights and respond to residents' requests for assistance in a timely and professional manner.
  • Document medications and treatments administered, as well as care provided, in a clear and concise manner.
  • Assist with orientation of new employees on the unit and participate in resident care planning.
  • Assess residents, document findings, and report to the physician as needed.
  • Transcribe physicians' orders and communicate resident change of condition to resident sponsors.
  • Maintain accurate records of nursing care provided and complete required documentation.

Requirements:

  • Current licensure as a Licensed Practical Nurse in the state of Indiana.
  • First aid, mantoux, and CPR certification.
  • Ability to understand and communicate with residents and complete nursing forms.
  • Ability to follow verbal instructions and provide nursing care practices.
  • Ability to use medical equipment within scope of licensure.
  • Ability to provide safe and efficient resident care, following all applicable state and Veterans' Administration rules, IVH policies and procedures, and all HIPAA rules.

Working Conditions:

The work environment is generally on a nursing care unit, in a controlled environment. The LPN may be exposed to body fluids and cleaning chemicals, as well as violent residents. The LPN must be able to walk and stand for long periods of time, bend, stoop, twist, reach above shoulders, push/pull, and assist with lifting and transferring residents, from 100 to 500 pounds, using a gait belt or other accepted practices, or with a mechanical device.

Skills:

  • Proficiency in computer skills and ability to learn other programs used in maintaining electronic medical records.
  • Hands-On - American Heart Association CPR and BLS Certification required.
  • Dementia 6-hour certification is required prior to start.
  • LTC experience required.
  • EMR, Matrix Care experience preferred.
  • Experience with Memory Care.
  • Experience with short-term rehab.
