We are seeking a skilled Home Health Nurse to join our team at Aveanna Healthcare. As a key member of our Care Team, you will play a vital role in providing high-quality care to medically fragile children and adults in their homes.
Key Responsibilities:- Implement and manage individualized care plans tailored to each patient's unique needs
- Conduct comprehensive patient assessments and coordinate care with other healthcare professionals as needed
- Administer medications, treatments, and therapies as prescribed by physicians
- Document all care provided in a timely and accurate manner
- Collaborate with patients, families, and caregivers to promote health education and self-care
- RN licensure in good standing; LPN/LVN license accepted with RN supervision
- CPR certification from American Red Cross or American Heart Association
- TB skin test within the last 12 months
- Six months of prior nursing experience preferred but not required
- Reliable transportation
$32-$35 per hour, plus benefits including paid time off, health insurance, and opportunities for career advancement.
