MyMichigan Health is seeking a skilled Care Manager RN to join our team. As a key member of our healthcare team, you will be responsible for providing comprehensive care management and coordination services to adult and pediatric patients with complex illnesses in the primary care setting.
Responsibilities:- Collaborate with primary care providers, patients, and healthcare teams to develop individualized care plans and implement targeted interventions.
- Identify high-risk populations and develop strategies to mitigate risks and improve health outcomes.
- Implement evidence-based clinical guidelines and protocols to optimize patient care and reduce healthcare costs.
- Provide patient education and support to empower patients and families to manage their health and well-being.
- Coordinate care across settings and facilitate transitions of care to ensure seamless patient care.
- Develop and implement improvement plans to address healthcare issues and promote quality and efficiency in patient care.
- 2 years of case management experience preferred.
- 3 years of experience in adult medicine/pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting within the past 5 years.
- Knowledge of chronic conditions, evidence-based guidelines, prevention, wellness, health risk assessment, and patient education.
- Health plan/Payer experience preferred.
- Critical thinking and problem-solving skills, with the ability to analyze complex data sets.
- Excellent assessment and triage skills, with the ability to implement evidence-based interventions and protocols for chronic conditions.
- Ability to communicate effectively, both verbally and in writing, and to work independently with minimal supervision.
- Associate's Degree in Nursing required; BSN preferred.
- Registered Nurse (RN) licensure in the State of Michigan required.
- Exposure to stressful situations, including those involving public contact, trauma, grief, and death.
- Able to wear personal protective equipment, including latex materials or appropriate substitutes, as required.
- Ability to move freely about the facility, with or without an assisted device, and to perform the functions of the job as outlined in the job description.
- Overall vision and hearing necessary, with or without assisted devices.
