We are seeking a skilled Speech Language Pathologist to join our team at Lone Tree Post Acute. As a Speech Language Pathologist, you will be responsible for developing and implementing effective treatment plans for patients with speech and language disorders.

Key Responsibilities
  • Develop and implement treatment plans for patients with speech and language disorders.
  • Treat patients according to physician treatment plans.
  • Communicate with supervisors and other healthcare team members regarding patient progress, problems, and plans.
  • Participate in patient care conferences, utilization review meetings, and rehabilitation conferences as needed.
  • Participate in in-services training programs for other staff members in the facility.
  • Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, and other required documentation.
  • Record treatment changes according to policy and procedures.
  • Instruct patient families or nursing staff in maintenance programs and caregiver training in preparation for discharge from therapy services.
  • Participate in discharge planning.
  • Act as a clinical preceptor for affiliating CFY/RFP Speech-Language Pathologists and student programs.
  • Comply with the Speech Language Pathology and Audiology Bureau State Practice Act and facility policy and procedures.
Requirements
  • Bachelor's degree in Speech-Language Pathology Therapy, preferably from a Master's or Doctorate program in Speech-Language Therapy.
  • Proficient in computer skills, including inputting data into programs such as Casamba and Point Click Care.
