Job Summary

We are seeking a Licensed Practical Nurse LPN to join our team on a night shift. As an LPN, you will be responsible for providing high-quality care to our residents, working collaboratively with our team to ensure their physical, emotional, and social needs are met.

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement individualized care plans to meet patients' unique needs
  • Collaborate with interdisciplinary teams to ensure seamless care transitions
  • Document patient information accurately and thoroughly
Requirements

To be successful in this role, you will need:

  • Active, current, unrestricted Licensed Practical Nurse LPN licensure in the state of practice
  • Excellent communication and interpersonal skills
  • Ability to work collaboratively as part of a team
  • Strong problem-solving and critical thinking skills
