Job Duties: Your primary job duties will include coordinating patient care with the medical team, interdisciplinary team, and external agencies. You will participate in daily IDT meetings to develop and review care plans, conduct routine health assessments to monitor participant well-being, assess and treat acute health concerns, manage chronic disease states across various care settings, lead Advanced Care Planning discussions, educate participants and families on chronic conditions, collaborate with community partners, participate in quality improvement initiatives, and provide on-call support as needed.

Education and Experience: To succeed in this role, you will need to possess current licensure and certification to practice as a Nurse Practitioner (NP) or Physician Assistant (PA) in the State of Michigan. A Master's degree in nursing or satisfactory completion of a physician assistant program is also required. You will need to hold BLS certification, current DEA registration, and State of Michigan Controlled Substance License. A minimum of 1-year experience working with the frail and elderly or long-term care population is preferred.
